1. Development Screening: I give permission of a developmental screening tool to be used with my child while in care within daycare. This will aid in my child receiving the best care possible.

2. Medical Aid/Assistance: In case of an emergency, I authorize the qualified staff of Smartkids Daycare to take any steps necessary for the benefit of my child.

3.Transportation: I understand that time to time KidsConnection Daycare may need to transport my child by walking/yellow school bus/ public transit to field trips and hereby authorize the staff to do so.

4. Field Trips: I understand that as a part of the program at the center my child will visit various community areas/amusement centers/ Library.I have read and understood the conditions provided above. By sending my child to Smartkids Daycare, I am agreeing to the conditions and consent to all actions taken by the staff for the benefit of my child.
